Every autumn the same question comes up in kitchens and pharmacies: does elderberry syrup really help against colds? A look at two decades of clinical research gives a clear answer: taken early, elderberry has been shown across several randomised trials to shorten cold and flu symptoms.
European black elderberry (Sambucus nigra) is one of the most familiar shrubs in Europe. It grows along forest edges, railway embankments and farmhouse gardens from Portugal to Scandinavia, and has flavoured syrups, teas and folk remedies since antiquity. Over the last two decades, this long tradition has been joined by an unusually consistent series of randomised trials.
Only the ripe, processed berry belongs on the table
An important point first: raw, unripe elderberries, as well as the leaves, bark and seeds of the plant, contain cyanogenic glycosides, chiefly sambunigrin. When eaten, these can release small amounts of hydrogen cyanide, which may cause nausea, vomiting and diarrhoea. Heating – cooking, or simmering into syrup or purée – breaks down sambunigrin and the likewise poorly tolerated lectins almost completely. Finished, processed elderberry products from retailers are not affected by this; the caution applies to raw, foraged berries.
Shorter colds: what randomised trials show
The best-known study comes from Zakay-Rones and colleagues in Norway: in a randomised, double-blind, placebo-controlled trial with 60 adults who had suffered from influenza A or B symptoms for no more than 48 hours, symptoms cleared on average four days earlier under elderberry extract than under placebo, and the need for rescue medication was significantly lower. A later placebo-controlled study in 312 long-haul air travellers between Australia and an overseas destination found shorter and milder cold episodes among those taking elderberry extract during the trip than in the placebo group – a setting where travellers tend to catch colds unusually often.
A 2019 meta-analysis pooling four randomised controlled trials with a total of 180 participants found a large effect in favour of elderberry for relieving upper respiratory symptoms. An independent review that included five clinical studies with 936 adults reached a similar picture: when an elderberry preparation was taken within 48 hours of symptom onset, cold or flu duration was often shorter, and many participants reported noticeable improvement in fever, headache and nasal congestion within two to four days.

What might be behind the effect
At the molecular level, most explanations point to elderberry's anthocyanins, the deep purple pigment group responsible for the berry's colour. In laboratory experiments, elderberry flavonoids have been shown to bind to surface structures of influenza viruses, limiting their ability to enter host cells; other in-vitro work describes an aqueous elderberry extract promoting the maturation of dendritic cells, the immune cells that trigger T-cell responses. A systematic review using GRADE evidence-quality assessment rates the overall strength of evidence as moderate and notes that elderberry appears to shorten an existing cold rather than prevent one.
Everyday use: who should take particular care
The available studies were conducted almost exclusively in adults; robust data on children are largely lacking, which is why standardised elderberry preparations should be used cautiously in children. For pregnancy and breastfeeding, there is currently insufficient scientific evidence to reliably assess use – caution and a conversation with a doctor or pharmacist are advisable during this period. Anyone taking medication that specifically suppresses the immune system (for example after an organ transplant) should check with a doctor before using elderberry preparations. In the underlying studies, adverse effects were generally rare and occurred more often in the comparison groups than under elderberry.
